A strong example

A weak example, with feedback

Weak: Two required boxes blank. Why and how to fix: Look for stars or the word required, and fill each one.
Weak: Signed, no date. Why and how to fix: Sign and date. A form without a date may be sent back.
Weak: Kit: four papers listed. Why and how to fix: List all seven papers and mark each have it or need it.
